Sharing your Assessment Package

In a nutshell

You choose when to share your child's Assessment Package. Sharing gives your clinician organised assessment preparation information, but it does not replace their own clinical assessment.

Before you share

Take a moment to review your Assessment Package.

Check that:

  • your child's basic details are correct
  • you have completed the activities you intended to complete
  • relevant reports have been added
  • you are comfortable with the information being shared

Your Assessment Package does not need to be perfect before it can be useful.

What does your clinician receive?

Your clinician receives an organised summary of the information collected through Threadline and any included supporting information.

The purpose is to help them review information from different sources more efficiently.

Good to know

Sharing your Assessment Package does not ask your clinician to accept Threadline's conclusions. Threadline does not make a diagnosis or clinical determination.

What happens after sharing?

Your clinician may review the package before or during your appointment.

They may still ask questions, conduct interviews, complete a medical assessment or request additional information.

That is a normal part of the assessment process.

You remain part of the conversation

An organised Assessment Package is not a substitute for speaking with your child's clinician.

You can still explain concerns, correct misunderstandings and discuss anything that has changed since the information was prepared.

Your child's clinician remains responsible for the assessment and all clinical decisions.
