In a nutshell

You can add relevant reports and supporting documents to your child's Assessment Package as they become available. Focus on information that helps explain your child's history or current functioning.

What can I upload?

Depending on your child's history, relevant documents may include:

  • school reports
  • psychology reports
  • speech pathology reports
  • occupational therapy reports
  • paediatrician or specialist letters
  • educational or learning assessments

You do not need to upload every document your child has ever received.

How to upload a report

Open the reports or documents activity within your child's assessment preparation journey.

Choose the file from your device and follow the prompts to upload it.

Wait until the upload has completed before leaving the page.

Check that you uploaded the right file

After uploading, check the document name and make sure it is the report you intended to add.

Good to know

Before uploading a document, check that it relates to the correct child and does not contain information about another person that you did not intend to share.

Can I add reports later?

Yes.

Many families find additional reports while preparing for assessment or receive updated information later.

You can add relevant documents as they become available.

What if I upload the wrong document?

Where the option is available, remove the incorrect file and upload the correct version.
