Your Assessment Package brings together the information you collect through Threadline into one organised summary for your child's clinician. It prepares information for assessment, but it does not diagnose ADHD or make clinical conclusions.
What can be included?
Your Assessment Package may bring together information such as:
- your child's developmental, medical and family history
- your concerns and examples from everyday life
- parent questionnaires
- teacher observations and questionnaires
- information about emotional wellbeing
- school and functioning information
- previous reports and assessments
Exactly what is included will depend on your child's circumstances and the information available.
Why is the information organised?
Assessment information often arrives in different formats and from different people.
A parent may complete questionnaires. A teacher may provide classroom observations. Previous reports may sit in separate files. Important examples can be difficult to remember during an appointment.
Threadline brings these pieces together so your clinician can review a more organised picture.

What your Assessment Package does not do
Your Assessment Package does not:
- tell you whether your child has ADHD
- replace a clinical interview
- replace a medical assessment
- recommend treatment
- make decisions for your child's clinician
Your child's clinician remains responsible for deciding what information is relevant, gathering any further information they need and making all clinical decisions.
You stay in control of sharing
Your Assessment Package is prepared within Threadline so you can review the information you have collected.
You decide when it is ready to be shared with your child's clinician.
The goal is simple: help your clinician start with the clearest, most organised picture possible.
